HAD03 indicates whether or not this facility is the provider's primary hospital affiliation. A "Y" indicates the facility is the primary affiliation; an "N" indicates the facility is not the provider's primary affiliation.

HAD04 indicates whether or not the provider has clinical privileges in good standing. A "Y" indicates the provider has clinical privileges in good standing; an "N" indicates the provider does not have clinical privileges in good standing.
